⨠NEW PATIENTS & EXISTING PATIENTS (INJECTABLES) â¨
NEW PATIENTS: All new patients must book a virtual NP (Nurse Practitioner) assessment appointment prior to booking injectable treatment.
EXISTING PATIENTS: As part of our commitment to safe, ethical, and responsible medical aesthetics care, all patients receiving injectable treatments are required to complete a yearly NP virtual assessment.
This appointment ensures that your:
- medical history is current
- treatment history is reviewed
- injection plan remains appropriate and up to date
Patients booked with Jessica, NP do not require a separate virtual assessment with Kelsie, NP.
đ˛ A $25 no-show fee applies to all unattended virtual consultations.
If the required NP assessment has not been completed prior to your injectable appointment, we will need to contact you to reschedule.
⨠No NP assessment is required for microneedling, laser treatments, HIFU, facials, or chemical peels â just come in and glow!
đ INTAKE FORMS
To ensure a smooth booking experience and respect for our injectorsâ time, all intake forms must be completed at least 24 hours prior to your appointment.
For existing patients, intake forms must also be updated yearly.
â° CANCELLATION POLICY
We require 48 hoursâ notice for all appointment cancellations.
Appointments cancelled with less than 48 hoursâ notice will incur the following fees:
- $50 for appointments 45 minutes or less
- $100 for appointments longer than 45 minutes
A valid credit card is required on file for all last-minute cancellations and no-shows.
