This appointment ensures that your medical information, treatment history, and injection plan are up to date for ongoing injectable care.

📋 INTAKE FORMS: To ensure a smooth process and respect for our Injectors’ time, we kindly request that all intake forms be completed at least 24 hours prior to your initial appointment. For existing patients, this intake form needs to be completed yearly.
⏰PLEASE NOTE - We require 48 hours notice to cancel your appointment. All appointments cancelled with less than 48 hours notice will be charged $50 for an appointment of 45 minutes or less and $100 for an appointment of 45 minutes or more. A credit card is required on file for last minute cancellations or no-shows
